Population Level Analysis and Community EStimates (PLACES) (www.cdc.gov/places/), a collaboration between the Centers for Disease Control and Prevention (CDC), the Robert Wood Johnson Foundation, and the CDC Foundation, began with providing model-based estimates for 29 measures among all counties and most incorporated and census-designated places, census tracts, and ZIP Code tabulation areas across the US.
PLACES now includes 9 SDOH measures from the American Community Survey, which may be useful to communities working to become more age- and dementia friendly that are working on local or regional action plans.
These newly added measures are poverty, education, unemployment, crowding, housing cost burden, internet access, racial or ethnic minority status, older persons, and single-parent households. These new additions to PLACES allow public health and community planners to identify overlapping health and social factors in their community to inform the allocation of resources and public health planning.
